New Patients

Before we begin the treatment process, we must first collect some information on your current condition and medical history. New patients will need to complete the "New Patient Information Form," to help us better understand their needs and provide the most effective treatment.

For all new patients, we will need to collect documents and will discuss health information regarding the patient’s current condition, insurance information, any current medications, medical history, family history, allergies, and any other pertinent information. This information will allow us to develop a unique treatment plan tailored to meet your individual needs. New patients will also need to bring their ID and insurance card.

To schedule an appointment for your initial consultation, call us at 480-739-6765, and one of our friendly team members will assist you and answer any questions you may have before your visit. We look forward to meeting you and helping you on your road to mental wellness.

Initial Examination

You probably have a lot of questions, especially if this is the first time at Push Wellness Health. One of the first questions many people ask is how long they will need to allow for the first appointment. Though it can vary between individuals and circumstances, we usually advise people to allow one hour for the first appointment. This first appointment is known as the consultation. The consultation is an opportunity for us to get to know you and for you to get to know us. The quality of the doctor-patient relationship is crucial to the success of treatment and is something we place a premium on. We will discuss any current struggles and how you are coping with them. Please be assured that anything you say in this appointment is confidential. The therapist will listen in an open and nonjudgmental manner, and you do not have to answer any questions you are not ready to. However, the more open you can be and the more information you can provide us, the better we will be able to help you. We will discuss a recommended course of treatment and set a time for your next visit. We reserve the last portion of the consultation for you to ask any questions you have of us and or our treatment process.

Pain Concerns

We emphasize patient comfort in everything we do here at Push Wellness Health. A large part of a successful treatment process is making sure each patient is comfortable. We want you to feel physically and emotionally at ease in our care. Treatment of any type can cause patients to feel anxious, especially if it involves any invasive tests or procedures. First, we will not order any test or treatment unless we feel it is crucial to helping you get better. A number of physical illnesses can manifest as symptoms of depression or anxiety. Thus, it is crucial for your health that we rule those out. Second, we will discuss any concerns you may have, look at all the possible ways we can address these concerns, and eliminate as much discomfort as possible. Lastly, we will walk you through any procedure you are unfamiliar with and explain its part in your care plan. Do not hesitate to bring up anything that you are anxious about so we can advise you and put you at ease.

Insurance

In an effort to serve as many patients as possible in the Phoenix and surrounding area, Push Wellness Health accepts a wide variety of insurance plans and providers. To find out if we work with your insurance, please call our office at 480-739-6765 for assistance. We will answer as many questions as we can regarding insurance coverage. However, you may need to contact your insurance provider for the specific details of your plan or additional information.

This is unfortunate, and we want to do our part to make this situation easier. As healthcare professionals, Push Wellness Health makes serving the health needs of our patients our first priority. If your insurance does not cover a portion of treatment, our team will work with you to find a solution.

Financial Policy

At Push Wellness Health, our overarching mission is to provide the highest quality therapy to patients who need it in Phoenix and the surrounding area. Being in good mental health is vital to all aspects of a person's general health. We believe access to mental health services is also a crucial part of the health of the community at large. Therefore, we strive to remove any barriers to accessing our mental health care services. No one should be left out or avoid seeking the care they need because of worry over how they will pay for it. We believe that everyone deserves top-quality care regardless of their financial situation. In an ideal world, everyone would have insurance, and all insurance plans would cover mental health services. However, we know that for many, this, unfortunately, is not the case. Still, we do not believe lack of insurance coverage should be a barrier to care either. Therefore Push Wellness Health makes every effort to make our services available to anyone regardless of their finances or whether or not they have health insurance. We accept a number of payment options, including cash, check, financing, and all major credit cards. In addition, we offer payment plans tailored to your budget. We can discuss these in further detail during the consultation. Our team will do all we can to work with you. If you have any questions about our financial policy, do not hesitate to call 480-739-6765. We look forward to helping you.
